|Description:||We serve individuals who are challenged by loss, poverty, and stigma associated with a diagnosis of mental illness or experience as a foster youth. Services include dual diagnosis treatment, mental health counseling, residential treatment, emergency shelter, case management, permanent supported housing, transitional housing, peer support, money management, and independent living support for former foster youth, and HIV testing and support for those with HIV/AIDS.|

RIVER STREET EMERGENCY SHELTER is a 32-bed emergency shelter for homeless adult men and women. Shelter staff work with residents to achieve self-sufficiency goals and connect with community resources for obtaining benefits, physical health services, employment, and housing.
The Money Management Program (MMP) provides representative payee services for individuals in Santa Cruz County receiving Social Security benefits who, because of their disability, need assistance in budgeting and managing their money.
Casa Pacific is a 12-bed residential program for individuals with mental illness and co-occurring substance abuse issues. El Dorado Center (EDC) is a 16-bed, community-based treatment program for individuals in need of intensive residential services. Transition House is a 10-bed crisis residential program for individuals in crisis who are in need of an alternative to psychiatric hospitalization.
Second Story is a six-bed house which serves as a respite and a voluntary opportunity for individuals to learn to use relationships to move out of old roles and patterns.
The Santa Cruz Community Counseling Center (SCCCC) and CFSC, its partner corporation, own and operate over 130 beds in duplexes, apartments, and multi-bedroom homes throughout the County in order to provide stable, permanent, affordable housing for individuals with psychiatric disabilities.
TRANSITION AGE YOUTH PROGRAMS (TAY):
The Independent Living Program (ILP) assists current and former foster and probation placement youth aged 15-21 to develop independent living skills and achieve educational/vocational goals to successfully transition to self-sufficiency.
Independent Living Resource Center assists current and former foster and probation placement youth ages 15-24 in building the skills, self-esteem, and support system necessary to make a successful transition to independent living in the community.
The Transitional Voucher Program (TVP) is a joint program where the Housing Authority provides Section 8 Housing Choice Vouchers to a small number of participants, ages 18-20, in ILP.
Transitional Housing Plus (THP Plus) is a supportive housing program that serves former foster and probation youth between the ages of 18-24. Participants receive assistance securing independent housing in the community and meet regularly with their THP Plus Coordinators to work on independent living goals.
THE ENHANCED SUPPORT SERVICES (ESS) Team provides support seven days per week to individuals open to Santa Cruz County Mental Health (CMH) who are in distress, to prevent or de-escalate a crisis, in order to avoid unnecessary hospitalization. The team has a regular schedule of 11:00 am to 9:00 pm daily, and is available on an on-call basis from 9:00 pm until 8:00 am daily. The team has three primary functions:
THE DOWNTOWN OUTREACH WORKER engages homeless individuals in the downtown Santa Cruz area and connects them to needed resources such as mental health or substance abuse treatment, social services, physical health services, financial benefits, transportation, meals, emergency shelter and longer term housing.
