|Description:||We are a local, one-of-kind grassroots organization. Our support services extend to families with children under 18 diagnosed with cancer within the counties of Santa Cruz, Monterey, San Benito and South Santa Clara. Jacob's Heart provides emotional, social and financial assistance to children with cancer and their families.
ELIGIBILITY: Any family with a child diagnosed with cancer before the age of 19.
|Mission:||To improve the quality of life for children with cancer and support their families in the challenges they face.
|Programs:||Case Management and Counseling, Direct Financial Assistance, Family Social Support Program and Community Awareness & Education.
Individual, Couple, and/or Family Support is provided for all immediate family members. These sessions can be scheduled with a Family Specialist as hospital visits and office visits, or home visits through the “HeART on Wheels” program.
Family Night Groups are offered one time per month.
Grief Support is provided for all ages and family members.
