Description:Health Projects Center (HPC), established in 1980 as a 503(c) non-profit organization, offers health-related and social programs to health professionals, older adults and family caregivers. Health Projects Center exists to address the health and human needs of individuals, families and communities of California's Central Coast by developing and implementing high quality programs and strategies.

ELIGIBILITY: Any member of the public may call for information and referral.

FEES: No charge for information, referral, needs assessment or family consultation. Sliding scale for respite and counseling service.

AREAS SERVED: Health Projects Center serves the Monterey Bay region of California, and hosts the Del Mar Caregiver Resource Center, Multipurpose Senior Services Program, and the Central Coast Area Health Education Center.

offers services for family caregivers (family members or friends) of adults living with a brain impairment such as Alzheimerís disease, Stroke, Dementia, Huntingtonís disease, Parkinsonís disease, Multiple Sclerosis, Brain tumors or brain injury, Mild cognitive impairment and other conditions that may cause memory loss or confusion

works to improve access to and the quality of health care for the medically underserved in Monterey, Santa Cruz and San Benito Counties. Central Coast AHEC builds community-academic partnerships for health education and health workforce training.

Helping older adults maintain their independence at home. MSSP offers free social work and nursing services to medically isolated, frail and underinsured older adults who are at risk for premature institutionalization or loss of independence. We provide care management services to assist clients with daily living needs so that they may live healthy, active lives.

Program Qualifications:
∑ Are 65 years of age or older
∑ Have monthly Medi-Cal eligibility with no share of cost
∑ Live in Santa Cruz County or Monterey County
∑ Willingness to participate in an individualized care plan
MSSP may be able to help you coordinate these services:

∑ Respite Care for Caregivers

No charge for services

Helping adults who have been living in a long-term care facility for over three months, move back into their own home or community. A CCT Coordinator will work with you to find out if there is an affordable place for you to live and what services are available to help you make the transition.

Volunteer Info:Minimum Age: 21. This non-profit organization helps caregivers cope with sometimes overwhelming responsibilities of being a caregiver. They need volunteers to do some light office work. Call for more information 459-6639
