Hospice of Santa Cruz County offers compassionate, expert medical care and grief support to patients and their loved ones facing the end-of-life. Caring for the body, mind and spirit, we support the unique needs of each patient in the comfort of their own home, in a nursing home or in a residential care facility. We are experts at symptom management so that individuals can live in comfort and dignity. We offer support to patients and their families 24 hours a day, seven days a week. Our Transitions program supports those with a life-limiting illness by easing certain concerns, creating care giving plans, and identifying community resources to assist with healthcare issues.
ELIGIBILITY: Determined case by case.
FEES: Hospice care is covered by Medicare, most insurances or a sliding scale.

|Programs:||CAMP ERIN: Hospice of Santa Cruz County is pleased to partner with The Moyer Foundation to present Camp Erin© of Santa Cruz County. Camp Erin is a FREE, weekend long grief support camp that is designed to help children and teenagers ages 6-17, who have experienced the death of someone close to them.
Our We Honor Veterans program is a partnership with the National Hospice and Palliative Care Organization and the United States Department of Veterans Affairs. Together we are committed to bringing comfort, dignity and quality of life to veterans in our community.

Our Children’s Grief Support program for children and adolescents offers individual, group and in-school support. Kids receive age-appropriate support and tools to process their grief in a healthy manner.
Support in the workplace for coping with a death or anticipated death.
A Lending Library of books and tapes on grief and loss.
Referrals to other community resources.
Grief support is available to everyone in our community. You do not have to receive other hospice services to take part in our grief support programs. If you are grieving the death of someone close to you, call us at (831) 430-3000 or email us. Let us share how we can help.
A Drop In Grief Support Group meets Fridays Noon-1pm in Scotts Valley.
Mid-County Ongoing Group: Tuesdays 6:30-8pm in mid-county, a new group starts every 3 months. Pre-registration is required. Please call 430-3000 for more information and check website for ongoing groups.

Become a volunteer at Hospice of Santa Cruz County and play an important and unique role in someone’s final days. Hospice Volunteer Visitors offer support to people facing the end of life. You may simply offer a hand to hold or run a few errands. You’ll also join a caring community of hospice volunteers.
Hospice Volunteer Visitors offer support to people facing the end of life. They’re understanding listeners who also make it possible for a caregiver to take a few hours of respite.
Anyone who feels moved to apply is welcome! We’re also looking for bilingual volunteers, art therapists, musical volunteers, veterans wanting to serve other military veterans, and massage therapists.
