Community Bridges-Elderday Health Care Center, an adult day health center founded in 1981, serves Santa Cruz County's adults and elders who need assistance with daily living tasks. The program helps participants reach and maintain their maximum level of physical, mental, emotional and social functioning by providing an opportunity for companionship, activities and professionally supervised therapies. Elderday's objective is to help participants maintain or improve their current level of functioning.

A nutritious morning snack and lunch is served daily.

Safe, reliable door-to-door transportation is provided in Santa Cruz County, in lift-equipped vans by Lift Line.

ELIGIBILITY: Functionally impaired ages 18 and over, elderly and those with dementia. For people who are striving to maintain their independence, people who are dependent upon others due to their physical impairment or mental confusion and families and caregivers needing respite from the daily responsibility and stress of caring for a loved one

FEES: The cost of Elderday can be covered by Medi-Cal, the Veteran's Administration, long-term care insurance, or private pay on a sliding scale.
Door-to-door transportation is provided by Lift Line, when needed, at no additional cost.

FUNDING SOURCE: County of Santa Cruz, Medi-Cal, Veteran's Administration and generous community donors - donations are welcomed and needed to enrich program services.
Programs:Enrichment activities: yoga, art, sing-a-longs, gardening, knitting, woodworking, and group celebrations or birthdays and holidays.

Certified Music & Memory program, bringing the joy and memory of personalized music into the lives of the people we serve.
Volunteer Info:Volunteers from the community bring music, activities and companionship to Elderday attendees, enriching the lives of the participants as well as the volunteers.
